[सत्र की तारीख और बंदी के ब्योरे, मामला संख्या और कैलेंडर वर्ष, मूल अपराध और दंडादेश की जानकारी, तथा दंडादेश के लघुकरण संबंधी न्यायालय के आदेश का ब्योरा कथन करें।]

आपको एतद्द्वारा प्राधिकृत और अपेक्षित किया जाता है कि आप उक्त ............... (बंदी का नाम) को अपनी अभिरक्षा में उक्त कारागार में, जैसा विधि द्वारा अपेक्षित है, तब तक सुरक्षित रूप से रखें, जब तक वह आपके द्वारा उक्त आदेश के अधीन आजीवन कारावास के दंड को भुगतने के प्रयोजन के लिए समुचित प्राधिकारी और अभिरक्षा को सुपुर्द न कर दिया जाए।

To the Officer In-charge of the Jail

[Recite the session date and prisoner particulars, case number and calendar year, the original offence and sentence, and the particulars of the Court's order commuting the sentence.]
